We all experience lapses in attention, concentration and memory. We forget what we went upstairs to retrieve. We need to re-read pages in books to refresh our memory. We often have names, places and details on the tip of our tongues but are unable to produce them. These lapses can be due to normal, age-related changes, but when cognitive problems begin to interfere with safety and the ability to care for ourselves and/or others, a neurological event or process may be the cause. When these changes become concerning, it is time to consult with healthcare professionals. Our Forward Thinking program seeks to address all of the mental processes involved in gaining knowledge and comprehension, including thinking, knowing, remembering, judging and problem solving. These are higher-level functions of the brain and encompass language, imagination, perception and planning.

We perform a collaborative needs assessment through a systematic analysis of facility:
