RehabCare Group, Inc. and its affiliates and related companies (collectively, “RehabCare”) are committed to protecting the privacy of data that you submit to this web site or to web sites under our operation and control (collectively, the “Site”).
RehabCare may ask you to provide information about yourself, or your companies, in order to better serve you, for instance, in connection with an application for employment. Examples of information requested may include name, title, company, address, phone number, email address, and other relevant data. RehabCare does not require this information to obtain access to any area of the Site. RehabCare does not wish to receive, nor should you submit, any medical information, information pertaining to any medical conditions, treatments, procedures, or providers of medical care to the Site. However, if you decide to provide personal information to RehabCare on a Contact Form, in no case will RehabCare sell, license or transmit that information to third parties. Contact Forms are used only to deliver requested information and respond to your questions. Any information that you voluntarily share with RehabCare in a form is kept strictly confidential and fully secure. When you submit a form, the encrypted (encoded) information is protected using “Secure Socket Layers (SSL)” as it passes between your browser and the RehabCare servers. Only authorized persons are permitted to access the information you submit.
RehabCare may compile statistical information concerning the usage of the Site. This information allows RehabCare to monitor its utilization and continuously improve its quality. Examples of this information would include, but not be limited to, the number of visitors to the Site, or to sections or pages within the Site, patterns of traffic flowing through the Site, length of time spent on the Site, or in sections or pages of the Site, the other sites that refer visitors to the Site, the pages of the Site that visitors frequently use as entry and exit points, utilization of the browser and operating systems and versions used by visitors to the Site, etc.
In order to compile this information, RehabCare may collect and store on a non-secure server, your IP address, your operating system version, your browser version, the pages you visit within the Site, the length of time you spend on pages within the Site, the site from which you linked to ours, search terms you used in search engines which resulted in you linking to the Site, etc. While all of this information can be associated with the IP address your computer had while you visited the Site, it will never be associated with you as an individual, or associated with any other information you may submit through the Site, or that RehabCare may store about you for any other purposes.
RehabCare may use web site “cookies” in the process of compiling the statistical information mentioned above and to tailor certain pages of the Site for your browsing convenience. You have the option of setting your browser to reject cookies, but our Site may not function correctly if you do. Refer to the Help feature of your browser for information on disabling cookies.
RehabCare collects the following information to measure the use of the Site and to improve that content of the Site: number of visits, average time spent on the Site, and pages viewed. RehabCare may retain this information on a non-secured server.
RehabCare may compile statistical results concerning the usage of the Site. RehabCare uses this information to prepare collective profiles of visitors’ activities for management and internal use. For example, RehabCare may track the number of visitors who view certain pages or use certain features of the Site. RehabCare does not disclose user-specific information.

RehabCare is committed to protecting the privacy and safeguarding the security of your protected health information. Each time you receive services from RehabCare or one of its affiliates (together, an affiliated covered entity), we record information that identifies you and relates to your medical condition, provision of health care, or payment for your treatment. Typically, this record consists of your medical history, symptoms, examination, observations, test results, diagnosis, care summaries, treatment, and future care plans. Understanding your health information and how it is used is important in maintaining its accuracy and confidentiality. This notice pertains to our workforce members and other health care providers we work with in a clinically integrated setting (e.g., members of our medical and clinical staff) and other participants in our organized health care arrangements, and pertains to uses and disclosures of your protected health information whether made verbally, on paper, or electronically, including through a health information exchange operated by RehabCare or a business associate.

We are required by law to maintain the privacy of protected health information, provide you with this Notice of our legal duties and privacy practices with respect to protected health information, and to notify you if you are affected by a breach of unsecured protected health information.
We are required to abide by the terms of this Notice while it is in effect. We reserve the right to change the terms of our Notice and to make the new Notice provisions effective for all protected health information that we maintain. If we change the terms of our Notice, we will make copies of the new Notice available to you and post a copy of the new Notice in a prominent location in our facilities and on our website.
Certain state health information laws and regulations, such as those dealing with mental health, HIV/AIDS or drug and alcohol records, may be more stringent that the federal privacy laws and further limit the uses and disclosures of your health information described above.
If you believe your privacy rights have been violated, you may complain to RehabCare or to the Secretary of Health and Human Services. You may file a complaint with the Privacy and Security Contact at the facility or location where you are or were receiving services or you may file a complaint using RehabCare’s Compliance Hotline at (800) 359-7412. You will not be retaliated against for filing a complaint.
