RehabCare’s Hospital Rehabilitation Services has been asked by more than 12 of its hospital partners to manage their skilled nursing/transitional care units. These programs offer medical management and less intensive rehabilitative care for patients with a much broader range of diagnoses. (Many of the services we provide to our partner facilities for IRFs are also provided for TCUs.)
Our ProMOS information management system is designed to track patient data through every venue of the continuum — including TCUs — lending continuity to performance improvement plans, monthly reporting and discharge planning and follow up. The standard corporate support provided to all of our units is available to TCUs.
VitalCare, a RehabCare company, provides specialty transitional care services for patients primarily in Southern California recovering from severe trauma to the head and spinal cord, and degenerative disease such as Amyotrophic Lateral Sclerosis and Multiple Sclerosis.
RehabCare prides itself on its ability to manage these units within the tight financial parameters that PPS imposes on TCUs. Many of our clients have found that our skill at moving patients through the continuum has increased the facility’s ability to keep these valuable units open, thus reducing the length of stay of TCU-appropriate patients on medical/surgical units.
