Health Care Plan
RehabCare understands that everyone has unique healthcare needs and for that reason offer three distinct medical plan options to colleagues. The Health Care Plan options each include coverage for medical, vision and pharmacy services, and include voluntary cost saving via preferred provider organization (PPO). Colleagues may select coverage for self, self and spouse/domestic partner, self and dependent children, or family.

Dental Plan
Dental coverage includes coverage for preventive, basic, major and orthodontia services. Colleagues may select coverage for self, self and spouse/domestic partner, self and dependent children, or family.

Flexible Benefit Plan
This plan allows colleagues to pay for certain medical and child/elder care expenses with before tax dollars thus lowering taxable income.
Colleagues choose a pre-designated amount of income, annually, to be withheld pre-tax through payroll deduction for use in paying medical expenses not covered by the health plan such as deductibles, co-pays, and for child/elder care. RehabCare contributes $10.42 per pay period ($250 annual equivalent) to established dependent care accounts.

Life and AD & PL Insurance
This company paid benefit provides colleagues with basic group life insurance and accidental death/ personal loss. The benefit is one times your annual salary.

Voluntary Life Insurance
Colleagues who wish to purchase life insurance in addition to that provided by the company may purchase coverage amounts up to 5 times annual salary. If the colleague purchases voluntary life insurance, spouse and dependents are eligible for voluntary life benefits. You can purchase up to $200,000 in coverage without Evidence of Insurability.

Short Term Disability Insurance
Short-term disability provides you with a portion of your income in the event that a disability prevents you from working. This voluntary benefit provides 70% of your weekly salary to a maximum of $1,500.00 per week.

Long Term Disability Insurance
Long term disability provides protection against the loss of your income in the event that a disability prevents you from working and earning income for an extended period of time. Coverage begins after you have been away from work for 26 weeks.

Long Term Care Insurance
This type of coverage is available on a voluntary employee paid basis for those who may be interested for self, spouse, parent, grandparent, parent-in-law or grandparent-in-law. The premiums are deducted from your payroll.

401(k) Retirement Savings Plan
Colleagues can reduce taxable income by saving and investing in the 401(k) plan. The company contributes to accounts based on the amount contributed by the employee. The account value grows untaxed until it is withdrawn.
The plan is a qualified plan subject to federal rules governing such plans. This plan allows immediate eligibility upon hire. Rollover funds from a previous employer's plan may be added at any time.
Any amount from 2% to 75% of pre-tax income may be invested in the plan. The plan offers investment choices and RehabCare will MATCH your contributions at fifty cents on the dollar for the first four percent of your income that is invested in the plan.
Colleagues become 100% vested in the amount contributed by the company after one year of service. Colleagues are automatically 100% vested in the value of their invested amount.

Paid Time Off
Scheduled time away from professional responsibilities for the pursuit of leisure and personal interests is part of a well-rounded professional's development.
RehabCare provides colleagues with a Paid Days Off (PDO) benefit that blends time off for all reasons (vacation, holiday, illness, bereavement separate plan, etc.) into one flexible system.

Company Stock Purchase Plan
Colleagues who wish to purchase company stock may do so through payroll deduction and in coordination with Computershare Trust Company. Purchase is made at market price, this plan is separate from the 401(k) plan.

Professional Education Assistance - Professional Choice Account
To enhance professional skill and patient care RehabCare supports professional growth through the Professional Choice Account "PCA". This plan allows individuals a lump sum to be utilized for CEUs, license renewal, national and state associations dues as well as subscriptions to professional journals.

Referral Bonus
Colleagues who refer a licensed therapist (full-time, part-time and on-call) who works for RehabCare for at least 90 days are eligible to receive a referral bonus.
